DESCRIPTION:Presenter: Dr. Laura Sidari \n\n\n\nThis informational webinar offers primary care clinicians and practice stakeholders an introductory look into foundational considerations for selecting models of integrated behavioral healthcare to best serve a target patient population. Participants will learn how to define integrated care and utilize a conceptual framework to direct service selection and effective care delivery. We will discuss integrated models commonly utilized in the United States\, with emphasis on Primary Care Behavioral Health (PCBH) and Collaborative Care Model (CoCM). Using a real-life narrative example of cross-model integrated care delivery\, participants will explore advantages and pitfalls to selecting care models for a given practice environment.

SUMMARY:Collaborative Care (CoCM) - Clinical and Financial Opportunities for Healthcare Organizations and Providers
DESCRIPTION:This webinar offers primary care providers an introduction to Collaborative Care education and the Collaborative Care model (an evidence-based model to identify and treat patients with depression and anxiety in primary care). In over 80 randomized controlled trials\, Collaborative Care was found to be more effective than care as usual in primary care settings. Collaborative Care has also been established as a more cost-effective way to manage treatment in primary care. Participants will learn about the patient-centered and evidence-based care practices of Collaborative Care treatment. \n\n\n\nPresenter: Dr. Virna Little is the Co-Founder of Concert Health and the co-founder of Zero Overdose. She is an internationally known speaker for her work in integrating primary care and behavioral health\, developing sustainable integrated delivery systems and suicide prevention. Dr. Little is an expert in CoCM and provides technical assistance to organizations and states around the country integrating primary care and behavioral health. She has been recognized with awards from the Society for Social Work Leaders in Health Care\, National Association of Social Workers\, Community Health Center Network of New York\, and others.
