Agenda Items/Topics Discussed: 

  • General check-in and partner roundtable
    • The group opened with a roundtable focused on wins from the past year of the CoCM project.
    • Participants described meaningful progress in spreading and sustaining the model across settings, including growth beyond early implementation sites, increased visibility of the work, and new clinics preparing to launch.
    • Several partners shared examples of momentum in their local settings, including beginning to serve patients, piloting new approaches, and strengthening team-based behavioral health support within primary care.
    • The discussion reflected continued enthusiasm for the project, appreciation for the opportunity to expand access to care, and recognition that collaborative care is becoming more established across participating sites.
  • CoCM brochure update
    • The group discussed the importance of using plain language so the brochure is understandable and useful for patients and families. The goal is to make sure the brochure is not only accurate for professionals, but also clear, simple, and easy for patients to understand.
    • The survey feedback helped identify the messages that resonated most strongly, including that CoCM makes behavioral health care easier to access through the primary care office, that patients do not have to manage depression or anxiety alone, and that the care team works together to support the patient.
    • Several clinics shared that the brochure would be helpful for aligning messaging across sites. Printed copies would be preferred, and a downloadable version that clinics can print locally would also be useful.
  • Future patient-facing materials
    • The group discussed the possibility of general posters for clinics. ABHIN had previously started this work and can revisit it once the brochure is finalized.
